Web form missing upload section

I recently updated Mayan to the current version (3.5.2), and now I can not use the Webform to upload documents.
The last box on the upload screen, where the local document path is defined, has disappeared from the screen.

I forgot to note that watch folders work fine, so the system for storing and processing files is all working.
I just can't use the web interface to upload files.

I really couldn't figure out what the issue was here, but I appear to have fixed it by upgrading my bare-metal installation to the current version (from the current version).
I followed all of the upgrade instructions, both the early version and the later versions.
Appears to have solved it.

Version 3.5.2 included a fix for jQuery. Based on the reports, we though it only affected the browseable API but is seems the Dropzone, the upload widget, was also affected.

We encourage our users to read the release notes before an upgrade, or even if they don't plan to upgrade, as the solution might be found there or a clue where to look next.
